Budget Share Remittance Forms & Instructions - Updated Annually.  The forms below have been updated and ready to use for 2010

Future FM World Missionary funds are to be sent directly to FMWM.  This does NOT apply to FMCNA Home Fair Share obligations that are to be sent to the Wabash Conference office.  See our World Missionary Support page for World electronic payment options or for mailing World Missionary Support, use one of these forms pdf form or for typing.

Instructions for preparing conference obligations

Conference obligations are due by the 10^th of the following month EXCEPT FOR All Health Insurance premiums.  Insurance premiums are due by the 10^th of the month PRIOR to coverage.

FMCNA Pension Information & Forms

Please keep us updated on any salary changes.  The Treasurer's Annual Pension Report is due every year.

All newly eligible persons need to enroll by completing:

Enrollment Form (PDF form)   AND one of the following…

ANNUAL Treasurer's Pension Report of Salary 

(This is an Excel form with auto calc.)

The report is due annually on Jan. 30 AND each time there is a change in salary.           OR

ANNUAL Treasurer's Pension Report of Salary 

(This is a PDF paper form to print and fill out)

Most of our forms are available in either PDF (for printing and manual completion) or Excel (with automatic calculations).  If possible, we suggest that you use the Excel forms and save them to your computer.  Please watch for annual updates.
