Instructions

PDF forms,  left click on the highlighted name.  The file will come up and simply choose the print function on the page.      (If you want to save the PDF form to your computer, use the same procedure for Excel forms below).

Excel forms and Lotus 123 forms,  right click on the highlighted name.  Choose "Save target as" and choose where you would like the file to be placed on your computer.  Choose "save" to complete the download.

Enrollment Form.  All new ministers need to enroll and return the form to the conference.      PDF form

2008 Pension  The report is due on Jan. 15 and each time there is a change to an appointed minister's salary.           Excel with auto calc.
